Transitions of care can be complex. Our experienced clinical and care coordination teams work closely with Doctors, Discharge Planners, and Case Managers to ensure continuity of care without delays or disruptions. We provide advanced pharmacy services in the right setting for each patient, reducing hospital stays, lowering costs, and improving comfort and outcomes.
Infusion Therapy
We provide home infusion services across all major islands, along with modern ambulatory infusion suites on Oahu, Big Island, Maui, and Kauai. Every therapy plan is reviewed by a pharmacist to ensure safety and appropriateness, with nursing support, patient education, and 24/7 clinical access available throughout treatment.
Specialty Pharmacy
Our specialty pharmacy supports patients receiving complex or chronic therapies with close monitoring for side effects and medication adherence. We coordinate with providers and insurers to manage prior authorizations and help patients access financial and copay assistance programs.
Long Term Care Medication Management
We provide medication management services for nursing homes, assisted living and skilled nursing facilities, hospice, and adult residential care homes. Our team conducts regular medication reviews to reduce errors, improve outcomes, and provide reliable delivery and responsive support for caregivers and care teams.
