Long term care medication management ensures that residents in nursing homes, assisted living & skilled nursing facilities, hospice, and adult residential care homes receive their medications safely and correctly.

What to Expect as a Patient of Pharmacare Hawaii

We’re here to make long term care medication management as easy and worry-free as possible for you. Here’s what you can expect as our patient:

A member of your care team will contact Pharmacare to initiate your enrollment in our medication management services. Depending on your care location, our team may request important details such as your date of birth, allergies, current medications, and insurance information. For hospice patients, this information is provided directly by your hospice team.

Our Pharmacy Technicians will then process your prescriptions and bill your insurance plan. If any medications are not covered or have high copays, we will notify you in advance so there are no unexpected charges.

A Pharmacist will carefully review your medication plan to ensure they are safe and appropriate for you.

Our delivery team will bring your medications directly to your care location - at no extra charge - so your treatment can begin without delay.

If you or your care team have any questions regarding your medication plan, a Pharmacist will be
available 24/7 to assist you.

Insurance Support

Our team communicates directly with your insurance provider to help keep your out-of-pocket costs as low as possible. We take care of the claims, approvals, and paperwork to make the process simple for you.

Blister Packaging

We offer easy-to-use blister packaging that organizes your medications by date and time, helping you take the right doses at the right times safely and confidently—at no additional cost.

Long Term Care Medication Management FAQs

Get answers to common questions about our specialized pharmacy services

We currently serve residents of licensed care facilities including: Assisted Living Facilities (include partners?), Adult Residential Care Homes, and Community Care Foster Family Homes.

To sign up for Pharmacare Medication Management Services, please fill out our Welcome Packet.

The Welcome Packet collects the essential information and authorizations needed to enroll a resident in our medication management services.

You may also find a hard copy at your community’s nursing station or give us a call at 808-840-5690.

Medications are delivered to your facility or home daily Monday through Saturdays.

Pharmacare LTC does not charge patients for their medication deliveries.

Medications may be packaged in easy-to-use Medication Blister packaging, easy-open vials, or child safety vials.

There are no extra charges for special packaging such as blister packaging.

A monthly invoice for any co-payments not covered by insurance will be sent to the responsible party on file. Payments may be made via check or credit card. For payment by credit card, please give our office a call at 808-840-5690.
